What does it do?Community mental health teams work closely with primary care teams to provide assessment,
care, support and treatment to older people with mental health needs
such as depression, anxiety, dementia and psychotic illnesses.

CMHTs provide an integrated (health and social care) service following
defined care pathways for functional (depression, anxiety) and organic
(dementia) illness. There is access to a wide range of therapeutic
interventions and support for carers.
