What does it do?Crossroads Care wants every carer to be recognised, supported and offered services to help them maintain their own health and wellbeing. Our aim is to relieve the stresses experienced by carers whose caring responsibilities may be due to a loved ones disability, illness or age. CCCEMT provides a range of high quality services including short breaks within the home, a palliative care service, support groups for those with a physical disability, early onset dementia or Aspergers Syndrome and a Young Carers project.
Who it is for: Provides services that respond to the needs of carers and the people they support, offering them peace of mind and understanding.
Where it is available:Local service covering: Congleton, Crewe and Nantwich, Macclesfield (Cheshire).
How to access or apply for it:
Crossroads Care Cheshire East, Manchester & Tameside operates an open referral system and we are more than happy to receive enquiries.
Carers can also access services from CCCEMT through:
Third party referral (family member or friend)
Direct Payments and Individual Budgets
What to expect then:
Once a referral has been made, the scheme will then contact you directly to arrange a mutually convenient time for our Care Coordinator or a senior member of staff to conduct an in-depth assessment with you at your home, or prior to hospital/hospice discharge, so that we can be sure we will be able to support you and meet your expectations of the service.
