Merton CMHT for Older People

What does it do?

The service provides integrated health and social care assessment and treatment for older people with mental illness. The service aims to
treat people in the community and in their homes where possible and can arrange any necessary support services from London Borough of Merton. The team can also provide access to day hospital and day services as well as admission to Crocus Ward on the Springfield Hospital site if in-patients care is needed. The team includes doctors, nurses, psychologists, occupational therapists and social workers.

Who it is for?

As a guide, this service is for people over the age of 65 with dementia or other organic illness and over the age of 75 with functional mental
illness such as depression.

What it costs:

The service is free of charge.

How to access or apply for it:

Referrals should be made in writing (letter, fax or email) by GP's or hospital medical staff. The referral letter must include a history of
both mental and physical health, details of the current condition and any risk factors (either to the patient or to others).
