What does it do?Hospital aftercare - Supporting you back to independence

We provide short term support (not personal care) and practical help for people 60+, who live alone in the Borough, who are being discharged from hospital. For many people, particularly those living alone, returning home after a stay in hospital or following an accident can cause anxiety.

Our aim is to support older people work towards self selected goals in order to regain their confidence and independence.

Referrals from health and social care professionals, older people themselves or their relatives. We also provide ‘Take Home and Settle’ from the Accident & Emergency Department - please note that we only accept referrals from hospital staff for ‘Take Home and Settle’.
