Overview of service
The Falls Prevention Service aims to reduce the number of falls within Shropshire County Primary Care Trust. We also aim to reduce the
number of possible fractures caused by falls.
We assess people who have fallen, are at risk of falling, or are fearful of falling.We offer a range of ways to lower an individual’s risk of falls and to help people stay healthy, active and independent.We may also signpost people to other teams and services within the community.We also raise awareness and provide training about falls to patients, health and social care professionals, private and voluntary agencies. This includes how to prevent falls and how to cope with daily life after a fall.
Who it is for?For people at risk of falling. The service is for people who are registered with a GP in Shropshire.
Where it is available?Local service covering: Bridgnorth, North Shropshire, Oswestry, Shrewsbury and Atcham, South Shropshire, Telford & Wrekin (Shropshire).
What it costs:The service is free of charge.

How to access the service
Access to the service is via the Care Coordination Centre (CCC) Falls Prevention Line. You can refer yourself to the service or be referred
by a health or social care professional.
