Our trained staff are available to help terminally ill people and their families make positive, informed choices about end of life care and to support the bereaved through a period of readjustment.

Hospital Aftercare Scheme - Rehabilitation
Our team of staff offer practical support in the home following discharge from a hospital ward. People must be aged 65 or above and finding it difficult to resume their activities of daily living. Support can be planned for up to eight weeks.

Providing information, advice and access to helpful, practical support and community services to all ages so that problems can be resolved before they turn into a crisis. Age Concern Lancashire's Nelson’s Reception is the face to face representative for Help Direct in the town working in partnership with Calico Enterprises Ltd. No appointment necessary just call in and speak to one of our receptionists. |
