What does it do?
Community care means helping people to live as independently as possible either at home, in the community or in short or long term care. Community Care Services include:

- equipment to help with daily living

- signing, minority languages, large print
- Health visitors
- District Nurses
- Self help groups
- Services from voluntary organisations
Who it is for?
These services are available to people with a wide range of needs including:
- Older people who are frail or vulnerable
- people with a physical disability
- people with a sensory disability (sight or hearing problems)
- people with a learning disability
- people with a mental health problem
- people with a drug or alcohol problem
- people with HIV and aids conditions
