What does it do?

The team offer support immediately before and following discharge from hospital to people with mental health needs over 65.

Packages can include information and advice, support to access benefits and services and support to get out and about in the community.

The aim of the service is to enable people to regain confidence and to pick up where they left off in their lives on admission to hospital.

For people over 65 with mental health needs who have been discharged from hospital.
