Show more "Southwold Road is an extra care scheme in Stoke Newington, for people over the age of 55 in need of housing with additional care and support and care due to dementia, physical disability or mental health issues.The scheme consists of 30 self-contained, one bedroom flats spread over three floors. Each floor has a communal kitchen, lounge and dining area. There are also two assisted bathrooms in the building, a specialist sensory room and an outdoor summer house.
Our support service offers you a housing related support service which can include: maintaining tenancies, advice on paying bills, setting up direct debits, apply for other benefits, linking in with healthcare and local community groups.
Every customer has a support plan that reflects their support needs and aspirations. Support workers regularly review these plans with you to make that they still reflect your needs.
Care is provided at the scheme by the Hackney local authority. The care is tailored to your needs and is available 24 hours a day, seven days a week.
We also offer a specialist social inclusion coordinator who regularly visits our London sheltered housing schemes to help arrange communal activities such as coffee morning and special events. This offers you the opportunity to keep active and socialise with other customers in the scheme.
Family Mosaic support staff are available Monday to Friday 8am – 4.00pm. Outside these hours there is an emergency pull cord system in each flat in case of an emergency." Show less
