Following a hospital stay or hospital discharge, elderly people may need help with transport to take them back home. The hospital may arrange this, or specific help may be available through local community transport, voluntary agencies, or volunteer groups. The hospital or local Community Transport are most likely to be able to provide wheelchair accessible transport, and ambulances are generally only available if the older person has special medical requirements. Older people are best advised to enquire at the hospital what transport if any would be available to take them home.
