How to access or apply for it:

Phone the Customer Liaison Team (for adult social care referrals, social work enquiries) on (01254) 587547, Monday to Friday from 8.45am to 5pm.

Phone the Care Network Hub (formerly 'Your Support, Your Choice' shop) on (01254) 292620 for help and advice on staying independent.
