How to access or apply for it: The service is only available by referral by a Healthcare professional and the forms must be completed and returned to Age Concern Herefordshire & Worcestershire for the client / patient to be accepted on the scheme. Once a patient is accepted onto the scheme appointments will be arranged by the Age Concern Herefordshire & Worcestershire office. Appointments will be automatically allocated at regular intervals after the initial appointment (usually every 8 weeks). |
