What does it do?

In order to support you when you are leaving hospital Glasgow City Council and the health services have developed a home care ordering system designed to allow you to leave hospital with support.

Some support at home services can be accessed via the ward staff.

What it costs:

There is a charge for home care services. This is based on income rather that the amount of service you receive.

As part of the assessment you will be offered a free financial assessment to ensure that you receive all appropriate benefits

If you are 65 years or over and have been in hospital for longer than 24 hours you will be entitled to free home care for up to four weeks.

If you are 65 years or over you will not be charged for personal care tasks.

How do I arange this service?

Before you are discharged from hospital the ward nurse will discuss with you any support at home requirements you may have. They will be able to access home care services, but where a range of services is required they will ensure that your situation is reported to the hospital social work team for an assessment of your needs.

What happens when I get home?

The Home Care service will start when you get home, at the level and frequency agreed with you and arranged by the ward staff.

A member of staff from your local social work office will visit you in your home shortly after your discharge from hospital to carry out a more detailed assessment of your needs to make sure the service is suitable for you. This assessment will confirm or adjust the level of service you receive.

What happens if the service ordered by the hospital does not meet my needs when I return home?

Before you are discharged from hospital you will be given an information leaflet with telephone numbers to contact if you experience any difficulties when you get home.
