What does it do?The team includes occupational therapy staff, assessment officers and sensory impairment workers. They provide assessment advice and guidance for disabled people and their carers who require assistance with activities of daily living. This can include equipment, minor and major adaptations and associated grant aid, and registration for people with a visual impairment.
The team has a rehabilitation officer for visual impairment, who can assist with independent living skills, mobility and communication. The assessment officer provides community care assessments for service users with complex needs who have a physical and/or a learning disability, the assessment will identify appropriate services.
Who it is for: You must:
a. be a permanent resident of the East Riding of Yorkshire
b. have a permanent disability
We will not be able to offer an assessment if you have a temporary/short-term difficulty in managing your daily activities
