What does it do?They assist individuals and their families to identify appropriate support to achieve their desired outcome. As part of the support planning process adult services staff will assist and advise on how the individuals could meet their outcomes, through a range of options.

Who it is for:

The adult care management team provides an assessment of need to eligible adults aged 18 and over.

What it costs:

A financial assessment will be undertaken by a member of the Welfare Rights team in order to determine whether an individual will have to contribute to the cost of any services provided following an assessment of need
