North Community Mental Health Team

What does it do?

The North Community Mental Health Team comprises nurses, a consultant psychiatrist, an occupational therapist, a social worker, a clinical
psychologist and a support worker; all disciplines offer varying amounts of time/sessions to the community mental health team.
The team works 9am to 5pm Monday to Friday. We offer a service to people over the age of 65 years with mental health problems and to younger service users with a diagnosis of early onset dementia in the north of Hillingdon borough. The majority of the team’s work is carried out in the service user’s home and outreach facilities in the north of the borough. We offer specialist assessments and ongoing care and support to service users and make, where appropriate, onward referrals. The community mental health team works closely with the inpatient areas at the Woodland Centre, voluntary and statutory services.
