End of Life Support
Our trained staff are available to help terminally ill people and their families make positive, informed choices about end of life care and to support the bereaved through a period of readjustment.

Hospital Aftercare Scheme - Rehabilitation
Our team of staff offer practical support in the home following discharge from a hospital ward. People must be aged 65 or above and finding it difficult to resume their activities of daily living. Support can be planned for up to eight weeks.
